Membership Application

Each family must complete the membership application form in its entirety. Please carefully read through before submitting. Membership in SJ4HHE is FREE. NOTE: Certain programs or activities may have a small fee.

* ALL steps below are required for membership. *

1) Review the SJ4HHE By-Laws: This is a condition of membership. Click HERE to review the By-Laws.

2) Complete the SJ4HHE application: After your completed membership application is submitted, received and approved, membership is good for the current school year but must be renewed annually. New members may sign up at any time during the school year.

CO-OP TEACHER: 1-4 classes/6 days in semester on designated days. Successfully complete volunteer leader application & background check. Attend all classes or contact admins & substitute if absence is unavoidable. Teachers receive a discount on Co-Op enrollment fees and may be reimbursed for expenses up to the designated class budget. SJ4HHE provides some class supplies & PE equipment.

CO-OP ASSISTANT TEACHER: 1-4 classes/6 days in semester on designated days. Attend all Co-Op days; assist teachers by passing out supplies; maintain class order; substitute in the event a teacher's absence is unavoidable. Assistants do not receive a Co-Op enrollment discount unless teaching another class.

CO-OP HALL MONITOR: 1-4 class periods/6 days in semester on designated days monitoring common areas for noise level, ensuring students are in correctly assigned classrooms during class time, notifying teachers of class end time, passing out or collecting cleaning baskets. Hall Monitors do not receive a discount on Co-Op enrollment fees.

CO-OP NURSERY LEADER: 1-4 class periods/6 days in semester on designated Co-Op days watching younger siblings while students ages 5-18 attend Co-Op classes. Nursery Leaders who are not teaching a Co-Op or PE class do not receive a discount on enrollment fees.

SJ4HHE ADMIN: Member in good standing for 6 months; successfully complete volunteer leader application, training & background check on 4-H online; work with other admins to coordinate & oversee programs; promote 4-H events; be a positive role model.

OTHER: I am not sure where I can help but will happily contact an Admin when volunteers are requested. I understand that SJ4HHE Admins may ask me to help with event set up or clean up including Co-op and other social gatherings, communicate group information to a field trip organization, generate ideas for community service, determine dates to host certain activities, etc.

3) Release of Personal Information: By clicking the Submit button below to apply for membership in Seymour Johnson 4-H Home Educators, you acknowledge that your name, email address, and other relevant information may be shared with SJ4HHE admins, coordinators, and instructors for the purpose of communication regarding activities, classes, supplies, or students, and to facilitate SJ4HHE programs. SJ4HHE may take photographs, videos, or interviews of students and families at SJ4HHE functions for use in the group's social media, website, or marketing. Full names will not be used unless SJ4HHE has a group publication, such as a yearbook.

4) Join our Facebook group: We invite you to join our Facebook group linked here. Our Facebook page is the quickest way to stay current on all our announcements & upcoming events, ask questions, etc.

5) Create a family 4-H Online account: Because we are a 4-H group, each family must create a family 4-H account at https://v2.4honline.com/ . Include each family member age 5 or older as well as parent volunteers. If you already have a 4-H Online account, please review and update it with any changes or revisions such as phone, address, child's grade level, etc.
